PATIENT RECORDS & COMPLAINTS

To request access to or a copy of records maintained by Sage & Silk Aesthetics, submit a written request that includes enough information for us to identify you and the records requested. Please include:
• Your full legal name and date of birth.
• Current phone number or other contact information.
• A description of the records or date range requested.
• How and where you would like the records delivered.
• Your signature and the date of the request.

For your privacy, we may verify your identity before releasing records. If you ask us to send records directly to another person or organization, additional authorization may be required. Requests will be handled within the time required by applicable law, and any copying fee will be limited to amounts permitted by law.
